COVID-19 Release of Liability and Safety Guidelines
Due to the on-going situation and seriousness of the COVID-19 pandemic, the following two items are required as part of the enrollment process for 2020-2021.
Release of Liability Form
- We require that you download and read the COVID-19 Handbook for Pakachoag Enrollees and Visitors as part of the enrollment process.
- If you are scheduled for in-person activity, please be sure to read the handbook in full in advance of your first visit, so that you are aware of all procedures some of which must be met prior to visiting the School or any of our satellite locations.
- We require that all enrolled families, adult students and anyone who will or might participate in any school activity sign the release of liability form, link posted below, as part of your enrollment or prior to your visit. Thank you.
